Child care licensing forms by state

Each state licenses child care through its own agency, and many publish their own enrollment, health and emergency forms. Here is every state's licensing agency, with a link to its official forms or licensing page and the date we last opened that link.

Not legal advice; confirm with your licensing representative.

All 50 states and DC

We have opened 44 of these 51 links, on the dates shown. The other 7 say "Link being verified" and name the agency without a link: our link checker hasn't been able to open those pages.

Have parents sign your intake packet on their phone

In CubbyFront, parents fill in and sign your enrollment intake packet on their phone, and every signature is saved with the time it was made. Families who prefer paper can still fill in your printed forms.

CubbyFront doesn't fill in your state's own forms. If your state publishes one, keep using it alongside the packet.

Questions

Does my state require its own enrollment form?
It depends on the state. Some publish forms that centers are expected to use; others list what a child's file must include and leave the format to you. Your state's page above lists its forms, and your licensing representative can tell you which ones your center needs on file.
Why do some states say "Link being verified"?
We link a page only after our link checker has opened it and found the agency's child care page there. Some state websites turn away every automated request, ours included, and may keep doing so. For those states the row names the agency, which is still the one to contact: search its name to reach its website.
How often are the links checked?
Each link shows the date we last opened it. We try every link once a month. A link we can't reach keeps the date we last opened it, and a link that's gone comes down.
